Build Clinical Instinct through Critical Thinking for Healthcare Education™ (CTHE).
Clinical judgment requires more than knowing what to do. Nurses must recognize changes in patient conditions, anticipate treatment needs, and determine which findings require immediate attention.
Critical Thinking for Healthcare Education™ (CTHE) develops these thinking processes through structured analysis, engaging instruction, and unfolding clinical cases.
A patient may appear stable until new symptoms emerge.
A familiar diagnosis may not explain the current findings.
A treatment plan may need to change as the clinical picture evolves.
Nurses must recognize relevant cues, identify clinical deterioration, anticipate treatment needs, and prioritize appropriate nursing interventions.
CTHE provides opportunities to develop the critical thinking behind these decisions through guided analysis, clinical application, and reflection.
Support nursing students as they develop clinical reasoning and judgment. Strengthen how faculty teach critical thinking, facilitate clinical learning, and use formative assessment.
Support clinical critical thinking across nursing practice and professional development. Strengthen how clinical educators guide reasoning, recognize learning needs, and reinforce clinical judgment.

For Nursing Students and Healthcare Professionals
Both development pathways are intended for colleges, universities, and healthcare systems.
CTHE combines structured critical thinking instruction with active learning and unfolding clinical cases. Participants examine patient information, recognize changes in clinical conditions, anticipate treatment needs, and reconsider priorities as new findings emerge.
How CTHE Develops Clinical Thinking
The learning experience follows a five-stage process that encourages participants to examine their thinking, apply the six analysis skills, and develop greater independence in clinical reasoning.
Assess current thinking tendencies and explore how the skill supports clinical reasoning.
Apply the skill to patient scenarios, examine clinical findings, and develop questions that guide reasoning.
Reassess patient information as conditions change, anticipate treatment needs, and reconsider clinical priorities.
Examine different reasoning approaches, participate in formative assessment, and use feedback to strengthen thinking.
Evaluate your reasoning, generate independent questions, and apply the analysis skills with increasing autonomy.
One of CTHE's distinguishing features is the use of unfolding clinical cases.
Rather than receiving all patient information at once, participants encounter new findings over time. Each development creates an opportunity to reassess the situation, reconsider assumptions, generate questions, and examine clinical priorities.
A patient is recovering from a recent hospitalization. Initial assessment findings appear stable, and discharge is being considered.
Participant focus: Examine the available information and identify what is known, assumed, or missing.
Additional assessment findings and laboratory results become available.
Participant focus: Identify relevant changes, relationships, and information requiring further investigation.
The patient develops new symptoms or a change in condition.
Participant focus: Reevaluate the situation, determine priorities, and identify appropriate next steps.
Additional findings require the participant to bring together information from multiple sources.
Participant focus: Generate questions independently, explain clinical reasoning, determine priorities, and reassess the response.
Guided by Dr. Elaine Lloyd's C.A.G.E.D. philosophy, CTHE recognizes that students, educators, and practicing healthcare professionals bring diverse backgrounds, experiences, and learning needs. This understanding informs our multimodal instructional approach, which uses varied teaching and facilitation methods to create multiple opportunities for engagement, understanding, and application.
Participants examine evolving patient conditions, anticipate treatment needs, and reassess clinical priorities as new information emerges.
Storytelling, mnemonics, music, and simulation provide varied ways to engage with clinical concepts and reinforce learning.
Educators observe participants' reasoning, identify learning needs, and provide feedback throughout instruction.
Participants develop their own questions, investigate clinical findings, and strengthen independent clinical reasoning.
CTHE applies B STATE's six critical thinking analysis skills to healthcare education and clinical practice.
Each skill develops a different aspect of how participants examine patient information, question assumptions, and approach clinical decisions.
1. Objective Analysis
Examine clinical information without allowing assumptions to determine conclusions.
2. Global Analysis
Understand the patient's overall clinical picture and anticipate potential treatment needs.
3. Detail Analysis
Recognize significant findings and changes in clinical data.
4. Perspective Analysis
Consider information from patients, families, and the care team.
5. Symptom-Source Analysis
Investigate possible underlying causes of a patient's condition.
6. Integration Analysis
Bring the analysis skills together to support clinical reasoning and judgment.
CTHE connects the six analysis skills to the cognitive operations of the NCSBN Clinical Judgment Measurement Model (NCJMM).
This instructional alignment supports the application of critical thinking to recognizing cues, analyzing findings, prioritizing hypotheses, generating solutions, taking action, and evaluating outcomes.
The CTHE instructional alignment is an independent educational interpretation. It has not been formally validated or endorsed by NCSBN.
